MANUAL THERAPY

Mobilisations of spine & peripheral
joints

For full and pain-free movement your joints need to move freely in all directions that they were designed. If they become locked or restricted it results in pain and stiffness. Nearly every restriction in movement can be regained by a specific joint technique performed by an experienced Musculoskeletal Physiotherapist. Joint stiffness can be associated with any joint, muscle, tendon and ligament injury. Plus, joints can become stuck open or closed. As a result, the joint treatment method used to correct your stiff or painful joints will vary depending upon your physiotherapist's diagnosis. After a diligent assessment, your physiotherapist will discuss with you what techniques would be the most effective, safe and pain-free.

Massage / Soft tissue release (myofascial release)

When muscles and tendons become damaged, impaired, knotted, tense or immobile, massage provides an effective healing treatment.

  • Massage encourages blood flow to the tissue being treated by stimulating the circulatory system. As the lymphatic system works in close proximity to the circulatory system, the massage will promote reduced pain and swelling by improving lymphatic flow.

  • Massage has a direct impact on the muscular system by stretching tight muscles, relaxing and releasing overused muscles, reducing spasms and muscular cramping, as well as assisting in tissue regeneration.

  • By working on the muscles around a joint, massage can increase joint range of motion and assist in increasing joint strength and stabilisation.

  • Massage can promote a decrease in cortisol, which is produced when we experience pain and stress. Massage also promotes an increased level of serotonin and dopamine, which improves your mood, relaxation and assists in relieving pain. This can also have a follow on effect by reducing anxiety, while enhancing sleep, concentration and energy levels.

Sport Massage

A massage style specifically designed for those involved in physical activity. Sports massage focuses on preventing and treating injury, while also enhancing athletic performance. Therapists will commonly use a mixture of myofascial release and deep tissue massage techniques. The massage therapist will commonly finish your sports massage with some gentle stretch therapy.

DRY NEEDLING

Dry needling is a Western adaptation of traditional Chinese Acupuncture. Thin, flexible needles are inserted into the body to achieve myofascial release. The technique is effective in treating back and neck pain, muscle strains, tight and painful scars, headaches, shoulder pain, and numerous of other problems related to the muscles, pain and the body coping mechanisms of these.

How dry needling works:

  • The stimulation caused by simply inserting a dry needle (no medication is injected) into a muscle triggers your body to release natural painkillers called opioids, and a feel good hormone called serotonin (neurological effect). These actively reduce your pain, assists in the healing process and thus enables you to function more effectively.

  • Stimulating a trigger point within a muscle with a needle releases the muscle and allows it work more efficiently by causing it to twitch/contract and relax. This reduces muscle pain and helps the muscle to contract and lengthen better, and will allow you to have more power and better flexibility in the muscle.

There is little risk associated with dry needling, and very few side effects, none of which are long lasting. The most common which include discomfort from the needling procedure, bruising, and occasionally a very small amount of bleeding, much the same as you would expect from any injection. On rare occasion people may experience temporary feelings of drowsiness, euphoria, tearfulness, sweating or chills. These are due to the release of your body’s natural painkillers (opiods/serotonin).

ELECTROTHERAPY

Ultrasound

There are three primary benefits to ultrasound.

  1. The speeding up of the healing process from the increase in blood flow in the treated area.

  2. The decrease in pain from the reduction of swelling and edema.

  3. The gentle massage of muscles tendons and/ or ligaments in the treated area because no strain is added and any scar tissue is softened.

These three benefits are achieved by two main effects of therapeutic ultrasound. The two types of effects are: thermal and non thermal effects. Thermal effects are due to the absorption of the sound waves. Non thermal effects are effects which result from the vibration of the tissue as the ultrasonic waves pass through them. These vibrations are transmitted in a way that directly stimulates cell membranes which make up the soft tissue. This physical stimulation appears to enhance the cell-repair effects of the inflammatory response. Studies have proven that Ultrasound helps in enhancing the metabolic activities of cells, and thus, in tissue repair, especially in soft tissue injuries.

TENS

TENS is a non-invasive, low-risk nerve stimulation intended to reduce pain, both acute and chronic.

There are two main methods that a TENS machine uses to provide you with pain relief: Pain Gating & Endorphin Release.

Pain Gating

A TENS machine produces millions of tiny electrical impulses, which enter your body via electrodes placed on the skin near or over the painful region. These fast electrical impulses override the slower pain impulses and travel quickly along your nerves to your spinal cord and up to your brain. Your brain is immediately bombarded by millions of pleasant electrical impulses. Your pain signals continue to be sent to your brain but travel along much slower (smaller diameter) nerve fibres. Since your brain can only interpret a limited amount of information, the pain signals are easily outnumbered by the pleasant high-speed TENS stimulation.

Endorphin Release

The Pain Gate can also be shut by stimulating the release of endogenous opioids (endorphins, enkephalins, and dynorphins), which are pain-relieving chemicals naturally released by the body in response to pain stimuli. Opioids are a naturally occurring hormone in the body. They are released in response to an injury or physical stress to reduce your pain and promote a feeling of wellbeing. Much like Morphine, and related medications, your opioids have a similar chemical structure, which explains their strong painkilling effects. TENS machines stimulate the release of these endogenous opioids. The opioids then inhibit the transmission of pain signals in the substantia gelatinosa part of the spinal cord - what is often referred to as the spinal root part of the nerve.

Interferentail Therapy

Interferential current therapy is an effective therapy used to relieve pain and accelerate the self-healing process, getting your body back to a healthy, pain-free state. The high frequency signals of an IFC penetrate through the skin into deeper lying muscle tissues. The Interferential Current device transmits electrical impulses in minute quantities through your skin. Underlying tissue and nerves are stimulated which begins the healing properties. Frequencies produced by the IFC have been proven to stimulate endorphins, the body's natural pain killers. This can help to create a self-healing process without the need to for medications. This form of therapy is also extremely useful in reducing pain, inflammation, curing oedema, and spasms.

REHABILITATION

Rehabilitation is a crucial part of the treatment process. It comprises of specific exercises prescribed by your physiotherapist to correct imbalances in muscle strength, to stretch tight muscles and to improve balance and proprioception, which is very important for the control of movement of joints. It is possible for your pain and dysfunction to recur if this critical aspect isn’t incorporated into the treatment process.

STRAPPING & TAPING

Different approaches of strapping are used to aid immobilisation of joints and providing external support or proprioceptive stimulation to areas of injury. This enables the body’s tissues to heal naturally. Examples include Kinesiotape (KT Tape) or Rigid strapping (Leukotape).

POSTURE CORRECTION

Posture is the position in which you hold your body upright against gravity while standing, sitting or lying down. Good posture involves training the body to stand, walk, sit and lie in positions where the least strain is placed on supporting muscles and ligaments. Good posture makes you look great, feel more energetic and project confidence. It also helps prevent injury and reduce pain. Many work related injuries are caused by sitting in the wrong position for long periods, chairs that are the wrong height or computers that are in the wrong position. This can lead to back or neck pain, and joint or muscle injuries. These are issues that can be altered very easily with the correct knowledge and the correct workstation setup.

CHEST PHYSIO

Chest physiotherapy is a group of treatments designed to improve respiratory efficiency, promote expansion of the lungs, strengthen respiratory muscles, and eliminate secretions from the respiratory system. The purpose of chest physiotherapy is to help patients breathe more freely and to get more oxygen into the body.

Chest physiotherapy includes postural drainage, chest percussion, chest vibration, turning, deep breathing exercises, and coughing. It is usually done in conjunction with other treatments to rid the airways of secretions. These other treatments include suctioning, nebulizer treatments, and the administration of expectorant drugs.
